Forms

On this page you will find a number of forms and links that will greatly assist you on starting or continuing on your scouting adventure.

  • Membership Application (online) – This link will take you to the Scouting page where you can submit an online application to join our pack. To begin, just click on “APPLY NOW”
  • Annual Health (A/B) Form – This form is required by all Scouts, Parents, and Siblings who will be participating in scouting activities and camping events lasting less than 72 Hours. Forms are valid for 1 year and need to be resubmitted every year.
  • Annual Health (C) Form – This form is for Scouts participating in Resident (Summer/Winter) camps or other camping events lasting more than 72 Hours.
  • Family Talent Survey – Our pack is made up of many talented people. By filling out this survey, you help identify the unique talents that you possess and can share with the pack.
